Background

The San Jacinto Methodist Family Medicine Residency Program is an ideal training ground for physicians who wish to become hospitalists. Because of adequate patient volume, we are able to expand the teaching adult medicine inpatient service to accommodate a hospitalist fellowship training service for at least 7 months of the year. Our residency program is known for its excellent patient outcomes, statistics in inpatient management for length of stay, and value of low cost per admission.

Goals

At the completion of the fellowship, the fellow will be able to:

  • manage efficiently all aspects of inpatient hospital care
  • achieve competence in invasive procedures usually performed by inpatient hospitalists
  • implement the principals of inpatient utilization management
  • move patients through the hospital system at the appropriate level of care
  • utilize consultant physicians in a medically appropriate and cost effective manner
  • manage a busy inpatient service keeping in mind cost effective care
  • prescribe medications in a cost effective manner utilizing formularies where appropriate
  • understand the role of the Family Physician in caring adult for ICU patients
  • lead quality improvement initiatives for the hospital

Objectives

  • Participate in monthly Independent Practice Association hospitalist meetings
  • Discuss utilization management on each patient with hospital case managers
  • Supervise residents on the inpatient team
  • Perform a utilization management review
  • Spend structured time with hospital administrative leadership and Case Management departments
  • Perform at least 10 intubations, arterial lines, and central lines to obtain competency in each procedure

Faculty

  • Family Medicine Residency Faculty will be assigned to the team in rotation: Dr. Clare Hawkins, Dr. Rebecca Hart, Dr. Louis Hughes
  • ICU faculty: Dr. Koshnevis, Cardiology faculty: Dr. Challa
  • Administrative faculty: Dr. Rebecca Hart

Academic requirements

  • Write a 10 page graduate level research paper on an inpatient practice management topic or hospital quality improvement project
  • Readings: Hospital Medicine 2nd Edition: Wachter (reading schedule) – text provided. Discussion of readings on a monthly basis with Director of Fellowship
  • Present three, forty-five minute lectures on a medicine, hospitalist management or ICU topic to the residents at didactic conference time

Benefits

  • Pay: $60,000 per year

  • Faculty will evaluate the fellow on a monthly basis using a similar evaluation form as used to evaluate Family Medicine residents. The fellow will be evaluated on the six competencies of professional behavior, systems based practice, clinical knowledge, communication, organizational skills, and business practices relating to a hospitalist.

Schedule

  • July 1-June 30, a 12 month fellowship
  • Hospitalist service – at least 7 months
  • One month self study month for preparation of research project.
  • Rotations: ICU – 2 months, Cardiology 2 months
  • M-F daily rounds on Hospitalist team
  • Daily Admissions

Call

  • One night per week as primary call for hospitalist group. If licensed, may perform at Jr. faculty level; take faculty call one night per week.

Requirements and Prerequisites

  • Texas license eligible
  • Graduate of a Family Medicine Residency in the US

Certification

  • Certificate of Completion after 1 year program completion
  • Texas State Board of Medical Examiners accredited
