Registration & Forms
We are so glad that your physician referred you to The Methodist Hospital Sleep Disorders Center. A physician’s order is required so please visit your physician to get a referral if you haven’t already done so! If you don’t have a physician, Dr. Aparajitha Verma, the Methodist Hospital Sleep Specialist, is available for appointments. Call 713-441-3961.
STEP 1: Schedule your sleep study. Please call Dana or Candy at 713-441-7854. We will do everything possible to accommodate your scheduling needs while still getting you your sleep study as quickly as possible!
STEP 2: Registration! We need to make sure we gather your demographic and insurance information before you come in to receive your study so that we can have everything prepared for you. You can do this one of two ways, the path you choose is up to you! One option is over the phone. Dana and Candy are available to take your information over the phone. Please give them a call at 713-441-7854. If you prefer, you are also welcome to use our Online Registration Form.
STEP 3: Please help your medical and technical team prepare for you by filling out the following forms. You can print these forms out and bring the completed forms with you to your study. You can also fill them out right on line! The path is up to you!
