The Skilled Nursing Facility of San Jacinto Methodist Hospital was established in 1987 for the benefit of patients undergoing some of the most difficult passages that anyone ever makes. With deep respect for our patients’ dignity, we provide sensitive and expert care at a time when health and life itself are very fragile. What is a skilled nursing facility?

A skilled nursing facility (SNF) cares for individuals who require rehabilitative services and/or the daily attention of licensed nurses. Such patients no longer need all of the medical support provided by a hospital, but they need more skilled care than they would have at home or in a nursing home. A SNF provides specialized nursing care and supervision 24 hours a day.

Activity Recreation Therapy Are SNF services covered by Medicare?

Medicare will cover 100 days of care per year, provided the patient remains eligible for coverage. The Medical Utilization Review Committee reviews all Medicare cases periodically, and no further Medicare payments will be made after the committee rules that a patient has ceased to quality for SNF care.

How is a person admitted to your facility?

Individuals are referred here by physicians, friends, family members, and organizations. In order to be admitted, however, patients must be under care of a local doctor, and they must have had a three-day hospital stay within the past 30 days; they must require care that involves skilled planning, observation, and implementation; or they must require skilled therapy on a daily basis.We evaluate each referral, and if a person does not qualify for care here, we will make an alternate suggestion.

How long do patients stay?

Our patients stay an average of 10-15 days, but there is no time limit as long as they qualify for SNF care.We do, however, try to make our patients as selfsufficient as possible so they can return home soon, or participate in alternate care. We start planning for discharge, if possible, as soon as they arrive.

The newly-remodeled San Jacinto Methodist Skilled Nursing Facility offers patient both private and semi-private rooms in a clean, cheerful environment.

Our comprehensive treatment team includes a dietician, physical therapist, occupational therapist, social worker, activity director, speech therapist, pharmacist, nurses, nurses aides, and physicians. Of special note is the fact that all of our aides have taken state boards in Geriatric techniques.

All of our patients receive special care. We respect their rights and we are sensitive to their social and personal needs. Our activity director helps them adjust to their situation and we make every effort to give families the time and privacy the want with their loved ones.

We are involved with our patient as their conditions change and they go home or return for further treatment. Our staff is constantly working with our patients’ physicians and a network of professionals for discharge or additional services. This practice ensures a higher quality of care and comforting knowledge of continuity.
